What is the aim of the service?

  • To ensure a visible entry point into Homelessness services. 
  • Ensure a consumer-focused and strengths based approach 
  • Coordinate access to Homelessness services 
  • Ensure collaboration and partnerships between agencies and the Department of Human Services 
  • Conduct Initial Assessment & Planning and Interim Response Level 1 

Criteria

 
  • cross target 25 and over 
  • all demographics (single, couple, families, with or without children) 
  • Homeless or at risk of homelessness 

Service Provided

  • Assessment to go on waiting list for crisis or transitional accommodation 
  • Provide accommodation options 
  • Provide emotional support in regards to housing and homelessness 
  • Advocacy 
  • Risk assessment and planning 
  • Provide Information and or advise 
  • Financial assistance Meal options 
  • Meal options 
  • Housing and homelessness services 
  • Links with and referrals into drug and alcohol, disability, mental health, and immigration services
